Declaration and Confirmation for Joining the “Alumni Association of Applied Physics, PolyU” (AAAP) as Life Member and Applying for the Federation of The Hong Kong Polytechnic University Alumni Associations (FHKPUAA) Card

I authorize the Department of Applied Physics (AP) of The Hong Kong Polytechnic University (PolyU) to:

- on my behalf, apply for the life-time membership of AAAP and FHKPUAA card; and

- transfer my personal information to the relevant PolyU departments and FHKPUAA to process my membership application.
I authorize PolyU (including its internal departments and constituent units) to use my personal information for updating my record at the University and maintaining future communication with me, including news and announcements of the University and other information (including information about activities, benefits and services, education programmes and solicitation of donations); and transfer my personal information to FHKPUAA and AAAP for processing my membership application.
I authorize FHKPUAA to use my personal information to maintain on-going communication with me, including the delivery of the announcement of the FHKPUAA and the relevant alumni associations, and other information such as activities, education programmes, benefits & services, and solicitation of donation, etc;

I hereby declare that I fully understand points 1 to 3 and confirm that I will apply for the life-time membership of AAAP and FHKPUAA card after graduation.

*AP will sponsor the life-time membership fee for joining AAAP and FHKPUAA.

We will send you the membership card when it is ready (the processing time will be taken 3 - 6 months which depends on the workflow of AADO).
